How to report a fatality, serious injury or dangerous occurrence to Occupational Health and Safety

Employers and contractors are required by The Occupational Health and Safety Regulations, 2020 to report all workplace fatalities, serious injuries and dangerous occurrences to Occupational Health and Safety as soon as reasonable possible.

Timely notification allows Occupational Health and Safety to respond appropriately to incidents where people were, or could have been, seriously or fatally injured.
